Newport Mental Health is seeking an experienced Case Manager to engage clients, reduce barriers, and support outcomes for adults with severe mental illness. The role manages a caseload across Newport County, focusing on recovery goals, care coordination, and access to housing and benefits.

Qualifications

• Associate’s degree required; bachelor’s degree preferred.
• 2+ years of experience in the Human Services field.
• Valid Driver’s License with reliable transportation.
• Excellent verbal and written communication skills.
• Strong organizational skills and deadline awareness.
• Self-motivated, independent and team-oriented.

Responsibilities

• Participate in daily team meetings to review client visits and coordinate care.
• Attend meetings with clients to develop recovery goals and plans.
• Support with medications, transportation, groceries, and benefits navigation.
• Communicate with client families and support networks as part of the recovery plan.
• Assist with housing needs and housing authority applications.
• Engage in supervision for therapeutic and problem-solving techniques.
