Job summary

Community Support Services seeks an AOD Community Rehabilitation Specialist (CRS) to join our case management team at the Commons at Madeline Park site in Akron, OH. This full-time role provides recovery-focused services to individuals with AOD issues and/or severe mental illness, acting as a service broker, coordinator, and advocate.

You will collaborate with clinicians, monitor assessments, support ISP development, connect clients to resources, and help residents comply with site rules while

Qualifications

• Bachelor’s degree in human services or related field required.
• Chemical Dependency license preferred.
• Experience with case management for individuals with substance use disorders required.
• Must have a car and valid Ohio driver’s license.

Responsibilities

• Coordinate and monitor evaluations and assessments for clients.
• Assist clients to self-manage AOD and psychiatric symptoms.
• Develop and implement an Interdisciplinary Service Plan (ISP).
• Connect clients with community resources and social supports.
• Accompany clients to appointments and support daily living skills.
