Clarvida is seeking a Recovery Coordinator for its Tehachapi location to provide outreach, information-referral, case management, and a full array of mental health services. The role includes coordinating treatment plans and monitoring progress with clients, families, and community resources.

The position requires a CA driver’s license, auto insurance, CPR certification, and at least 1 year of case management or psychiatric treatment experience.

Qualifications

• Proof of valid California driver's license and auto insurance, as well as proof of education are required; CPR certification
• Minimum one year equivalent prior case management or psychiatric treatment experience
• May be required to have knowledge of rehabilitation model
• Excellent prioritization and organization skills
• Strong interpersonal skills and good written and verbal communication skills
• Intermediate level PC skills required
• Related experience: coaching, scouts, teacher's aide, or CPS aid
• Closely monitored with six months of training

Responsabilités

• Conduct intake and evaluate client life situation, and with client consent, prepare a detailed plan of care
• Coordinate services by assisting clients in implementing their plan and linking to community resources
• Provide professional support with regular contact including home visits, guidance, and intervention as needed
• Transport clients using personal vehicle as required
• Monthly Productivity requirement is 100 hours
• Maintain daily records and billing forms per company and government standards
• Develop and maintain professional relationships with agencies and service providers
• Participate in meetings and training activities
