Job summary

Catholic Charities Archdiocese of St. Louis, Queen of Peace Center, seeks a Community Support Specialist to deliver recovery-oriented rehabilitation and support services for individuals with substance use and behavioral health needs.

Qualifications

• Bachelor's degree in Social Work, Counseling, Psychology, Human Services, Behavioral Health, or related field.
• Optionally four years of qualifying experience in behavioral health, social services, or related field.
• Valid driver's license with reliable vehicle.

Responsibilities

• Develop individualized recovery and treatment goals with clients by identifying strengths, needs, skills, and barriers.
• Develop, implement, monitor, and revise treatment and rehabilitation plans.
• Support clients in building natural supports and community connections for long-term recovery.
• Provide rehabilitation services to restore life skills, coping strategies, and healthy habits.
• Collaborate with interdisciplinary team to coordinate services related to housing, employment, education, and daily living.
• Deliver education, coaching, and experiential training in life skills and recovery management.
• Link clients to community resources, assist with referrals, and advocate for access to care.
• Coordinate and monitor services with external agencies to ensure continuity of care.
• Respond to crises by coordinating resources and facilitating access to services.
• Conduct outreach to re-engage clients who miss appointments and support ongoing participation.
• Assist with discharge planning and transition to ongoing supports.
• Maintain accurate documentation and meet productivity and billing expectations.
• Transport clients using a personal vehicle when appropriate and authorized.
• Participate in evening, weekend, and team coverage schedules.

Skills

Valid driver's license

Education

Bachelor's degree in Social Work, Counseling, Psychology, Human Services, Behavioral Health, or related field

Associate of Applied Science (A.A.S.) in Behavioral Health Support

Four (4) years of qualifying experience in behavioral health or related field
