Job summary

S:US in New York is seeking a Rehabilitation Specialist to engage people with mental illness and help them gain skills for community living and healthy independence. You will link clients to services, support recovery, and promote self-determination.

The role includes case management, developing service plans, crisis intervention, advocacy, and collaboration with therapeutic teams to ensure quality care in a supportive environment.

Qualifications

  • Knowledge of Microsoft Office tools.
  • Ability to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibility.
  • Basic knowledge of human services.
  • Bilingual (Spanish/English) speaking preferred.

Responsibilities

  • Provide case management to assigned caseload.
  • Develop and monitor individual service plans in conjunction with agency consumers.
  • Assist agency consumers with accessing needed community services through referral networks.
  • Assist agency consumers with problem solving and crisis intervention.
  • Accompany agency consumers to medical, psychiatric, and public benefit appointments.
  • Advocate for consumers and support self-advocacy.
  • Identify problems in daily living skills and provide hands-on assistance or referral.
  • Maintain strong working relationships with therapeutic support services.
  • Document all services in compliance with agency standards.
  • Support moves, lifting, packing and cleaning as needed.
  • Work with the social service team to develop off-site activities for community inclusion.
  • Liaise with community services and groups; participate in supervision and meetings.
