Job summary

Open Sky Community Services in Worcester, MA seeks a Recovery Coach to provide justice-system based and recovery-oriented supports, education, and safety planning for individuals enrolled in the program.

You will serve as a point of contact for integrated care coordination, collaborate with a multidisciplinary team, and connect participants with medical and behavioral health services, transportation, and community resources.

Qualifications

• High School Diploma, GED or equivalent, required.
• Minimum of two years experience working with adults with justice system involvement, co-occurring mental health and substance use disorders, required.
• Experience with local resources, navigating health, behavioral health and/or substance abuse treatment systems strongly preferred.
• Valid Driver's License, acceptable driving history and reliable transportation, required.

Responsibilities

• Engage with members as they enter the treatment system and support the members in gaining access to treatment services and community resources.
• Complete all Assessments and Support Plans according to prescribed timelines and specifications as given by Master's Level Clinician.
• Develop a supportive Treatment Plan with the member, in collaboration with the Master's Level Clinician that guides services in a way that is consistent and supportive of other existing comprehensive care plans.
• Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources.

Opportunity for Change is a partnership between Open Sky and the Office of the District Attorney in Worcester County. The program is designed as an upstream approach to supporting individuals with recovery and offers participants an alternative to prosecution and incarceration by participating in drug diversion program, which upon completion offers the opportunity for criminal charges to be dismissed. The program provides assessments, care plan development, and service referrals to ensure all necessary supports are in place for success. Candidates for the program are referred from the ten district courts located within Worcester County, which include Milford, East Brookfield, Gardner, Winchendon, Uxbridge, Leominster, Fitchburg, Westborough, Clinton, and Dudley.

The Recovery Coach will provide justice systems-based supports, recovery based supports, education, support, and safety planning for individuals. They serves as a point of contact for integrated care coordination and care transitions in collaboration with individuals enrolled, internal multidisciplinary team, justice involved agencies, medical and behavioral health providers, pharmacies, various social service agencies and other agencies and supports as indicated.
