Job summary

HealthLinc is seeking a recovery support professional to provide education and navigation across the recovery journey. You will collaborate with partners to visit patients in the community and ensure follow-up support for sustained recovery, including transportation to MAT appointments.

The role emphasizes detailed data coordination, timely communication, and strong engagement with individuals and families in Indiana communities.

Qualifications

• Must have a High School Diploma.
• Knowledge of methadone, buprenorphine, and naltrexone treatment options.
• Experience with addiction and/or mental health required.
• Lived experience preferred.
• Proven ability to coordinate, review, classify, prioritize, analyze, and present data.
• Proven ability to set priorities and to meet deadlines.
• Ability to communicate effectively on an individual or group basis.
• Valid driver’s license required for transportation to appointments.

Responsibilities

• Provides recovery education to patients across all recovery phases.
• Works with law enforcement and emergency services for home visits.
• Maintains follow-up contact to support ongoing recovery.
• Utilizes community resources to assess service appropriateness.
• Transports patients to MAT appointments when needed.
• Assists patients in articulating goals, steps, and barriers to recovery.
• Supports patients in identifying strengths for daily living and recovery skills.
• Observes progress and reinforces recovery goals.
• Supports vocational choices and helps with job-related anxieties.
