Job summary

Qualifications

• Four-year degree or equivalent experience in related field.

Responsibilities

• Provide case management and peer recovery support to participants.
• Assist participants in identifying goals, needs, and barriers.
• Connect participants to housing, food, transportation, healthcare, employment, and education services.
• Advocate to improve access to needed services and supports.
• Maintain confidentiality and professional boundaries.
• Complete documentation, referrals, and discharge summaries in a timely manner.
