Job summary

Qualifications

• High School Diploma or equivalent required; Bachelor's degree preferred.
• 1+ year of related behavioral health experience preferred.
• CPR certification and de-escalation training required.

Responsibilities

• Coordinate care across providers to implement the individualized service plan (ISP).
• Maintain the client’s clinical records and progress toward recovery.
• Begin discharge planning at admission and assist with aftercare.
• Coordinate referrals and AHCCCS applications as applicable.
• Communicate with treatment team and family to support goals.
