Job summary

Rushford Center, part of Hartford HealthCare, seeks a Case Manager Non-Clinician to help individuals with mental health and substance use disorders achieve wellness and independent functioning. You will assess needs, plan, and link to services within an integrated recovery-oriented system.

Qualifications

• Associate's degree in a related field is required.
• Bachelor's degree in mental health or human services preferred.
• Minimum of 2 years of experience with mental health or substance use disorders.
• Requires current, valid Connecticut driver's license.
• Able to qualify for Public Service License is preferred.

Responsibilities

• Assess client needs and develop treatment goals with the client and clinical team.
• Present case reviews to Treatment Team to create a multidisciplinary Treatment Plan.
• Provide crisis intervention when necessary.
• Support clients with group modalities to meet treatment goals.
• Orient clients to level of care, rights, and program policies; plan for discharge and maintain records.
