Job summary

DeCoach Recovery Centre in Fairborn, OH is seeking a Case Manager to provide direct client services, linkage to resources, and development of daily living skills to support personal recovery goals.

This role may offer flex hours to meet client needs; strong communication, advocacy, and collaboration with the Treatment Team are essential to coordinate care and community-based services.

Qualifications

• Graduate from an accredited college/university or high school.
• Minimum of one year experience in a behavioral health or related field.
• Active Ohio Driver's license with a clean driving record.

Responsibilities

• Provides case management under clinical supervision to adults with substance use and mental health disorders.
• Services are community based and require the use of a company vehicle that is properly insured.
• Requires excellent communication skills to help facilitate client care.
• Obtain individual and group case management services, community activities, facility visits, vocational training, and coordination of care as per the Treatment Plan.
• Comply with DeCoach Rehabilitation Centre policies and procedures.
