Job summary

Safe Harbor Recovery Center in Portsmouth, Virginia is seeking a Case Manager to join our treatment team onsite. You will coordinate care across providers, support discharge and aftercare planning, and maintain ongoing communication with the treatment team.

The role requires collaboration with families and referral sources to ensure adherence to the individualized service plan. The position offers a competitive hourly rate and requires CPR certification, a high school diploma or equivalent, and

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s degree in Social Work or related field preferred.
• Experience in behavioral health or case management preferred.
• CPR certification required.

Responsibilities

• Develop and implements appropriate activities within scope of practice.
• Implements services to clients per individualized service plan (ISP).
• Collaborates with client and family to implement service plan with options.
• Maintains comprehensive clinical records and progress toward recovery.
• Coordinates care with clinical team and external providers.
• Begins discharge planning at admission and follows up.
