Job summary

Applegate Recovery Services is seeking a dedicated Case Manager to support therapists and enhance the treatment process in an outpatient setting. You will coordinate communication among therapists, families, and referral sources while assisting in administrative tasks and treatment planning.

The role emphasizes professional interaction with clients and families, attention to detail, and the ability to participate in supervisory discussions and client independence goals.

Qualifications

• Experience working with adults with addiction is preferred.
• Must have a CDCA.
• Interact in a pleasant and professional manner with staff, physicians, providers, patients, and families.

Responsibilities

• Act as liaison between therapist, referring agency, and family; arrange visits and transportation.
• Ensure ongoing communication between family, referral source, and Mahajan therapeutics.
• Attend court hearings as needed.
• Prepare treatment reports for staffing with accuracy on client response.
