Job summary

Pinnacle Treatment Centers is seeking a dedicated Case Manager to support patients in addiction treatment. You will work with the Clinical Director and Executive Director to foster a therapeutic environment and coordinate care across multidisciplinary teams.

Qualifications

• Bachelor’s degree in the Human Service field from an accredited college or university.
• Valid/Current Driver's License.
• Ability to travel up to 15% may be required.

Responsibilities

• Providing case management and care coordination services for patients enrolled in treatment.
• Coordinating assessments, treatment planning and crisis intervention services; linking to community resources; monitoring service delivery.
• Assisting patients in obtaining services (housing, financial assistance, medical, educational, vocational, etc.) and coordinating with family and community networks.
• Coordinating treatment and discharge activities; ensuring timely team communication and collaboration within the facility.
• Maintaining confidentiality and adhering to policies; attending trainings and team meetings.
