Job summary

Personalized Nursing LIGHT House in Madison Heights seeks a qualified Full-Time LPN or RN to support a Men’s Residential & Withdrawal Management Program. The role is Monday–Friday, 7:00 AM–7:30 PM, in a 30-bed setting with a multidisciplinary team.

Responsibilities include monitoring residents, conducting health assessments, CIWA, medication administration support, and documenting care. Preference for substance use disorder treatment experience and trauma-informed care.

Qualifications

• Current LPN license required; RN preferred.
• Strong interpersonal and communication skills.
• Ability to maintain healthy and appropriate professional boundaries with clients.
• Basic computer skills, including Microsoft Office 365 and FileMaker Pro.
• Ability to pass a drug screen.

Responsibilities

• Monitor the medical status of residents, including vital signs, medication monitoring, and withdrawal symptoms.
• Conduct health assessments for each client admitted to the program and assist with nursing care plans.
• Observe and monitor clients’ physical, emotional, and behavioral status using CIWA and clinical protocols.
• Monitor and support clients in self-administration of medications and maintain records.
• Document behaviors, symptoms, observations, and incidents following protocols.
• Engage clients in supportive conversations about treatment readiness and recovery.
• Develop rapport and maintain therapeutic relationships with clients.
• Facilitate intake, orientation, and gather required information.
• Collaborate with counselors, peer recovery coaches, case managers, and team members.
• Follow health and safety policies and maintain a clean, safe environment.
• Assist with household tasks including meals, laundry, and light housekeeping.
• Perform other related duties as assigned by management.
