Job summary

Lutheran-Social-Services is seeking a Recovery Care Coordinator I in Fort Wayne, IN. This full-time professional position focuses on coordinating care for clients with addiction, delivering home- and community-based case management, and building relapse-prevention skills.

You will partner with a program manager and multiple community and agency partners to provide referrals, documentation, and timely care coordination. A driver’s license is required for community-based work.

Qualifications

• Bachelor’s degree in social services or related field preferred.
• Certified Addiction Peer Recovery Coach or Certified Peer Support Professional preferred.
• Minimum of two years’ experience working with addiction/recovery preferred.
• Valid driver’s license and car for full-time business use.

Responsibilities

• Engage individuals and families in therapy; complete mental health/substance use assessments.
• Provide short- and long-term case coordination, stabilization, and support services.
• Conduct screenings, assessments, and documentation in data systems.
• Coordinate care with housing, hospitals, mental health, and substance use programs.
