Job summary

High Point Inpatient Campus in Plymouth, MA seeks an Aftercare Coordinator to provide direct care, assessments, discharge planning and ongoing aftercare referrals for residents with substance use and mental health needs.

The role requires strong communication, organization, and collaboration skills, plus knowledge of EHR systems and HIPAA requirements. Part-time hours and competitive pay are offered, with opportunities for professional development.

Qualifications

• Must present as courteous and professional at all times
• Demonstrated flexibility and ability to perform multiple tasks
• Excellent communication skills, including organizing ideas in logical and clear fashion
• Knowledge of basic computer skills and ability to learn HPAO Electronic Health Record (EHR)
• Ability to work with others in cooperative and collaborative manner
• Understanding of relationship between substance use and mental health issues

Responsibilities

• Maintain assigned caseload.
• Provide direct care to residents – assessments, counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group notes.
• Make referrals when appropriate.
• Maintain medical records for residents in a professional, complete, timely manner.
• Link clients with aftercare services prior to discharge and assist access to services.
• Strengthen partnerships within the community to sustain sobriety and housing.
• Review patients’ rights and safety plans and grievance policy.
• Involve family in treatment as requested.
• Attend unit meetings and mandatory trainings.
• Continued education in the field relevant to service level.
• Coordinate with insurance for authorization when needed.
• Respond to codes and utilize campus responders.
