Qualifications

Responsibilities

• Conduct group and/or family sessions under supervision and document as required
• Co-facilitate assigned sessions and assist with modifying problem behaviors
• May perform initial assessments and follow-up assessments for patients
• Prepare individual treatment plans for assigned patients
• Ensure timely, clear documentation of patient care and incidents
• Liaise between referral sources and patients; provide crisis intervention as needed
• Plan aftercare for assigned patients; coordinate with insurance carriers
• Provide case management and evaluate needs for referrals to other programs or facilities
• Ensure patient safety and address complaints professionalism at all times
