Job summary

Mentor Community Services, part of the Sevita family, is seeking a Recertification Coordinator in Stockton, CA. You will handle annual contractual reviews for Host Home Providers and support the matching team by reporting homes with availability, while maintaining compliant records.

Qualifications

• Bachelor’s degree in human services with two years of related experience preferred.
• Equivalent combination of education and experience acceptable.
• Meet all state-specific regulatory requirements.
• Valid driver’s license, car registration, and auto insurance.
• Strong attention to detail and organizational skills.
• Ability to multitask and meet deadlines.
• Effective communication skills to manage relationships.
• Commitment to quality in everything you do.

Responsibilities

• Responsible for the annual contractual review process for Host Home Providers.
• Provide information to the matching team on homes with availability for placement.
• Maintain and audit files regularly for compliance.
• Work with the management team to develop and implement a Mentor retention plan.
• Coordinate ongoing Mentor appreciation activities and develop continuing education sessions.
