Job summary

Cera Care Hampshire is seeking a Reablement Support Worker to help people regain independence after hospital discharge. You will support rehabilitation, coordinate care with the reablement team, and provide compassionate, patient-centred assistance in the community.

Full training is provided through the Cera Academy and you can benefit from the Mobility Scheme, BHN discounts, Blue Light Card eligibility, a refer-a-friend scheme, and industry-leading career pathways.

Qualifications

• Experience in care or community support preferred.
• Ability to support rehabilitation and independence.
• Compassionate, patient and good communication.

Responsibilities

• Support patients leaving hospital and assist with rehabilitation.
• Collaborate with reablement team to plan daily activities.
• Provide emotional support and ensure safety during visits.
