White Oak Rehabilitation and Nursing Center in Toledo, OH is seeking a dedicated RD Registered Dietician to join our healthcare team. This part-time role requires 6 hours on-site with the remaining time remote, offering a flexible schedule and a supportive team.

You will develop personalized nutrition plans, perform nutrition assessments, educate residents and families, and collaborate with the care team to optimize patient outcomes in a nursing home setting.

Qualifications

• RD credential required and maintained.
• Experience in nursing home settings preferred.
• Ability to educate patients and families on nutrition.

Responsabilités

RD Registered Dietician Responsibilities

As an RD Registered Dietician, you will be responsible for the following:

• Developing and implementing personalized nutrition plans for patients with diverse dietary needs and preferences

• Conducting nutrition assessments and evaluating patient responses to nutrition interventions

• Providing education and counseling to patients, families, and caregivers on nutrition-related topics

• Collaborating with the healthcare team to develop and implement effective treatment plans

• Maintaining accurate and detailed records of patient progress and nutrition plans

• Participating in quality improvement initiatives to enhance patient outcomes and satisfaction

RD Registered Dietician Requirements

• Registered Dietician (RD) certification

• Prior experience in a nursing home as a Dietician preferred
