Job summary

White Oak Rehabilitation and Nursing Center in Toledo, OH is seeking an RD Registered Dietician for a part-time role (2 days per week). You will develop personalized nutrition plans, perform assessments, educate patients and families, and collaborate with the care team to improve resident outcomes.

Flexible scheduling is offered to accommodate needs. Requirements include RD certification; prior nursing home experience is preferred.

Qualifications

• RD certification required.
• Prior nursing home Dietitian experience preferred.

Responsibilities

• Develop and implement personalized nutrition plans for patients with diverse dietary needs.
• Conduct nutrition assessments and evaluate patient responses to interventions.
• Provide education and counseling to patients, families, and caregivers on nutrition topics.
• Collaborate with the healthcare team to implement treatment plans.
• Maintain accurate records of progress and nutrition plans.
• Participate in quality improvement initiatives to enhance outcomes.
