Job summary

Delta-T Group is seeking a Rapid Rehousing Case Manager to provide on-site support in the New Haven, CT area. This role is a contract opportunity (1099) with standard weekday hours (Mon-Fri 8:30 - 4:30) and opportunities to connect clients with housing resources, Medicaid, subsidies, and employment services.

The ideal candidate has housing case management experience across supportive, transitional or rapid-rehousing programs, with a Bachelor's degree preferred.

Qualifications

• Bachelor's Degree strongly preferred; however strong housing case management experience is more important.
• Case Management experience in housing programs including Supportive, Transitional, Rapid-Rehousing, etc.

Responsibilities

• Identify the needs of clients and establish social services support for housing.
• Identify eligibility for housing and assist with housing voucher applications.
• Assist with finding and securing apartments for clients.
• Screen documents for the homeless population, including sensitive information.
• Identify documentation for basic needs and entitlements (Medicaid, subsidies).
• Initiate referrals for mental health and addiction treatment services as needed.
• Encourage social supports and community connections.
• Assist with employment support including resume writing, job searching, and interviewing skills.
• Provide ongoing case management after housing is secured to support continued success.
