Job summary

PedIM Healthcare is seeking a proactive and compassionate HCC Coder to join our Primary Care team. This role is critical to accurate risk adjustment, complete documentation, and CMS guideline compliance while supporting high-quality patient outcomes.

You will review medical records, identify RAF/HCC diagnoses, ensure documentation integrity, and collaborate with providers and clinical teams to optimize RAF scores and coding accuracy across the care continuum.

Qualifications

• Must have experience in HCC/Risk Adjustment coding.
• Strong knowledge of CMS risk adjustment methodology and Medicare Advantage models.
• Credentialing in CRC/CPC/CCS or equivalent preferred.

Responsibilities

• Review medical records to identify RAF/HCC opportunities.
• Ensure accurate ICD-10 and HCC coding and documentation integrity.
• Collaborate with Providers and Clinical Teams to optimize RAF scores.
