Job summary

Concordia Visiting Nurses is seeking an experienced OASIS RN to join our in-office clinical team in Pennsylvania. You will review OASIS assessments and support staff to ensure accurate, complete documentation and Medicare compliance.

You’ll collaborate with clinicians, stay current with OASIS requirements, and contribute to high-quality patient care in home health settings.

Strong knowledge of OASIS, documentation standards, and EHR systems is required, with emphasis on detail and communication.

Qualifications

• Current, valid Pennsylvania RN license and home health nursing experience preferred.
• OASIS experience required and strong knowledge of Medicare regulations.
• Excellent attention to detail, organizational and communication skills.
• Ability to work independently while collaborating with clinical staff.

Responsibilities

• Review OASIS assessments for accuracy, completeness, and consistency.
• Ensure OASIS documentation supports patient’s clinical condition and plan of care.
• Review documentation for compliance with Medicare and home health regulations.
• Identify discrepancies, missing information, and opportunities for improvement.
• Collaborate with clinicians to clarify and correct OASIS documentation when needed.
• Provide guidance and education to clinical staff regarding OASIS requirements and documentation standards.
• Review assessments for appropriate coding and clinical consistency.
• Support accurate patient outcomes and reimbursement through thorough OASIS review.
• Stay current with changes to OASIS guidance, Medicare requirements, and home health regulations.
• Maintain timely and accurate documentation within the electronic medical record.
• Work collaboratively with the clinical team to promote quality, compliance, and positive patient outcomes

OASIS Registered Nurse Position

What You’ll Need

• Current, valid Pennsylvania Registered Nurse (RN) license
• Home health nursing experience strongly preferred
• OASIS experience required
• Strong knowledge of OASIS assessment requirements and Medicare regulations
