Job summary

Lumira Healthcare, formerly Livingston Community Health, is seeking a Quality and Population Health Specialist to lead quality improvement and population health initiatives at Livingston Community Health. You will drive performance, embed a culture of quality, and ensure alignment with HRSA requirements and value-based care programs.

You will oversee data integrity, dashboards, and reporting, coordinate audits and action plans, and collaborate with executive leaders to integrate quality

Qualifications

• Bachelor’s degree in a healthcare or related field.
• Minimum 2 years of healthcare quality improvement experience in an FQHC or similar setting.
• Knowledge of UDS, HEDIS, PCMH and value-based care metrics.
• Experience building dashboards and analyzing data.

Responsibilities

• Lead the Quality Improvement and Population Health Plan development and updates.
• Oversee data integrity and reporting, including dashboards and UDS submissions.
• Prepare for HRSA OSVs, FTCA deeming, and accreditation readiness.
• Coordinate provider peer reviews and credentialing support.
