Must be a local RN to Newark, Delaware or Philadelphia, PA.

St. Francis at Home is seeking a Quality Performance Specialist/Education Specialist-RN in Newark, Delaware for a Hybrid position.

  • Required Certification: OASIS Certification (COS-C or HCS-O) required within 6 months from date of hire. Must maintain recertification with no lapse in certification period.

POSITION PURPOSE: Identifies specific performance improvement initiatives and works collaboratively with the local agencies/leaders in their development, implementation and evaluation of quality improvement programs. Performs workflow review and completion to achieve standardization and organizational efficiencies. Ensures compliance with new regulations and standards in EMR documentation.

ESSENTIAL FUNCTIONS

  • Identifies, plans, implements, and evaluates agency quality assurance/quality improvement/process improvement programs, aiming for top decile outcomes, patient satisfaction, and strategic goals.
  • Supports in-service education and other continuing education activities related to quality, outcomes, QA/PI. Monitors publicly reported outcomes and other data using quality software and EMR reports.
  • Ensures agency staff compliance with federal/state and accreditation regulations through record review, case conferencing, mentoring, and survey readiness activities.
  • Oversees quarterly and annual agency QA reporting on QAPI and quality program initiatives.
  • Reviews discharge, death at home, and other OASIS documentation for accuracy and completeness, recommending corrections as needed.
  • Leads agency MDQI program, developing and implementing QAPIs and action plans to attain top decile outcomes and compliance.

MINIMUM QUALIFICATIONS

  • Registered Nurse with current license from respective State Board.
  • Bachelor’s Degree required.
  • Four years' experience in homecare; at least 2 years in a supervisory role preferred.
  • Certification: OASIS Certification (COS-C or HCS-O) required within 6 months of hire; must maintain recertification.
  • Valid Driver’s License and reliable transportation.
  • Proficient with computer programs, videoconferencing software, and telephone systems.
  • May require some travel.
