Quality Improvement Specialist – Nursing Home Expertise

Full-time, Remote Employment

Who We Are:

Constellation Quality Health is a non-profit health care quality consultancy and QIO-like Entity certified by Centers for Medicare and Medicaid Services (CMS) founded by physicians in 1983. Headquartered in North Carolina’s Research Triangle, we offer an array of quality improvement, clinical review, audit, technical, and consulting services and solutions to improve care delivery, system performance, and patient outcomes.

What You’ll Do:

We are seeking an experienced professional in nursing home operations and quality improvement to join our team as a Nursing Home Quality Improvement Specialist. This role is ideal for individuals with a background in nursing home administration, clinical leadership, or corporate support, who have a deep understanding of nursing home operations driving quality outcomes.

As a Quality Improvement Specialist, you will consult with nursing homes and skilled nursing facilities (SNFs) on initiatives supporting operations, implement evidence-based strategies and drive measurable improvements in care delivery.

  • Serve as a trusted advisor to nursing home providers, offering insights that drive sustainable, high-impact improvements in care quality and operational efficiency.
  • Assist nursing home leaders (Administrators, Directors of Nursing, Quality Directors) in assessing performance gaps and developing Quality Action Plans (QAPs).
  • Assist in technical assistance (TA) delivery, including data review, quality improvement coaching, and resource coordination.
  • Interpret clinical performance data to help guide providers in implementing improvement strategies.
  • Maintain accurate and timely records of quality improvement efforts using designated data systems.
  • Engage with healthcare organizations to provide guidance, education, and support for best practices.
  • Serve as the primary point of contact for healthcare providers, delivering responsive communication, problem-solving, and engagement to support quality improvement initiatives while ensuring provider satisfaction and program success.

This position is contingent upon contract award and works remotely; however, some travel within the southeast region may be required. Candidates in the Southeast region are preferred.

Our requirements for this role:

  • Experience in nursing home leadership or quality improvement in roles such as Administrator, Director of Nursing, Administrator-in-Training, Corporate Quality Consultant, or related positions.
  • Bachelor’s degree required (preferred fields: healthcare administration, nursing, public health, or related discipline); Master’s degree preferred.
  • Strong understanding of CMS regulations, Five-Star Ratings, QAPI, and federal/state survey processes.
  • Proven success in leading quality improvement initiatives that resulted in measurable improvements in resident care and facility performance.
  • Analytical skills to assess performance data and develop targeted improvement strategies.
  • Exceptional communication and leadership skills, with the ability to engage and educate nursing home executives, clinical teams, and corporate stakeholders.
  • Preferred but not required: Nursing Home Administrator (NHA) license or Administrator-in-Training (AIT), Registered Nurse (RN), or Certified Resident Assessment Coordinator (RAC-CT).

Why Constellation Quality Health?

  • Work in a mission-driven organization dedicated to enhancing nursing home care and outcomes.
  • Collaborate with industry leaders and experts to implement meaningful quality improvement initiatives.
  • Opportunity to shape operational and clinical excellence in post-acute and long-term care settings.
  • Remote flexibility with regional travel to engage directly with providers and support on-site improvement efforts.

This role is an excellent opportunity for experienced nursing home administrators, clinical leaders, and corporate quality professionals to drive meaningful change in nursing home care and operations. If you have the expertise and passion for improving resident outcomes and facility performance, we’d love to hear from you!
