Job description

About myPlace Health

As a PACE (Program of All Inclusive Care for the Elderly) organization backed by SCAN Group, myPlace Health brings together customized medical care, social activities, and daily support for participants and their families – all under one roof. Our centers are more than healthcare facilities. They are vibrant community hubs where participants are known by name, valued, and supported as whole people. Behind that experience is a dedicated, interdisciplinary team working together to coordinate care, remove barriers, and improve quality of life for some of the most medically and socially complex populations.

For employees, myPlace Health offers the opportunity to do deeply meaningful work in a highly collaborative setting. Team members are encouraged to contribute innovative ideas, and grow alongside a mission that prioritizes compassion, respect, and impact.

The result is a culture where people feel connected—to their colleagues, their participants, and the communities they serve. At myPlace Health, work is more than a job. It’s a shared commitment to honoring what matters most.

As a Quality Improvement Coordinator, you’ll play a vital role in supporting our commitment to excellence by working alongside the National Director of Quality and Compliance Programs. You’ll help drive local and enterprise-wide quality initiatives, ensure smooth adherence to federal and state reporting processes, and serve as a key contact for incident reports, root cause analyses, and the implementation of corrective action plans. In this role, you’ll collaborate with diverse stakeholders to uphold compliance with federal, state, and High Reliability Organization standards while embodying the values, culture, and mission that make myPlace Health a special place to work and grow.

Responsibilities
  • Data Management and Accuracy: Proficient in collecting, entering, and managing data for timely submission, ensuring reports meet quality and compliance standards.
  • Root Cause Analysis and Problem-Solving: Skilled in conducting thorough analyses to identify root causes, assess corrective actions, and drive improvements in quality processes.
  • Policy and Procedure Expertise: Acts as a knowledgeable resource for quality functions, including grievances, appeals, and facility standards, guiding team members on accurate application.
  • Attention to Detail: Executes and completes tasks related to quality improvement activities with precision, ensuring a high level of accuracy in all areas.
  • Collaboration and Communication: Effectively collaborates within QI Program Committees, actively participating and contributing valuable insights while maintaining detailed records, such as meeting minutes.
  • Analytical Reporting: Identifies and communicates deficiencies, anomalies, and trends found during quality activities, providing data-driven insights to support continuous improvement.
  • Regulatory Compliance: Supports audit preparation and compliance efforts, ensuring successful outcomes for state and federal audits and adherence to regulatory standards.
  • Commitment to Quality and Standards: Diligently collects and submits data for regulatory bodies and associations, reflecting a dedication to high standards and excellence in participant care.
  • Contribute to Organizational Growth: Embrace additional opportunities and responsibilities as needed, actively participating in the continued growth and success of the organization.

  • Bilingual in English/Spanish (test will be administered to assess proficiency).
  • Knowledge of healthcare compliance, especially in PACE programs, and familiarity with HIPAA regulations.
  • Self-motivated, self-directed professional who can confidently execute tasks in a dynamic environment, make sound decisions under pressure, and work independently with minimal supervision.
  • Strong multi-tasking and prioritization skills; proven ability to manage multiple tasks and balance competing priorities in a fast-paced setting.
  • Prior experience working in early-stage or rapidly growing companies is advantageous.
  • Experience within a PACE (Program of All-Inclusive Care for the Elderly) setting is highly valued.
