Responsibilities

• Review patient charts to identify care gaps and opportunities for patient outreach.
• Maintain and review patient registries for preventive care and chronic disease management.
• Support population health initiatives and value-based care programs.
• Help track and improve clinical quality measures and outcomes.
• Review payer information and use payer portals to identify/close quality gaps.
• Participate in payer meetings to maximize quality improvement opportunities.
• Assist with audits and follow-up related to quality improvement activities.
• Educate staff and providers on quality measures and improvement efforts.
• Participate in Continuous Quality Improvement, Chronic Disease, and Risk Management activities.
• Occasionally provide Medical Assistant coverage in Primary Care when needed.
