Job description

Quality Improvement Coordinator

Department: Quality Job Status: Full Time

FLSA Status: Non-Exempt Reports To: Clinical Quality Manager

Grade/Level: Amount of Travel Required: 40%

Work Schedule: Positions Supervised: None

8 a.m. to 5:30 pm Monday- Thursday.

8 a.m. to 12:00 pm on Friday.

However, adjustments in schedule or additional hours may be required to complete duties.

POSITION SUMMARY:

The Quality Improvement Coordinator is part of the care team and under the supervision of the Clinical Quality Manager. The Quality Improvement Coordinator will work in conjunction with all the health services within the health center in order to develop a comprehensive service delivery systems aimed at meeting the needs to the patients and assist in meeting organizational goals. This position demands the ability to interact with federal, state, and local agencies in order to maximize resources, and to coordinate program development goals. The Quality Improvement Coordinator is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.

SPECIFIC DUTIES AND RESPONSIBILITIES:
  1. Facilitates the development, implementation, and communications of programs or projects to promote preventative screening, population management, and high risk population management.
  2. Assist in collection and entry of patient specific data into the EMR.
  3. Hold an active role in the Quality Improvement program assisting to identify prevention, treatment, and/or management intervention opportunities to improve the quality care of services rendered.
  4. Ensure patient confidentiality.
  5. Ability to properly process data and compile reports on various projects.
  6. Notification and recall of patients regarding missed appointments, care caps, satisfaction, etc.
  7. Process requests from patients and staff via electronic messages.
  8. Maintain hospital tracking log; scheduling the necessary follow-up and obtaining necessary medical documents.
  9. Completes transition of care outreach including reconciliation of medication, adherence to care plans and treatment goals, and assistance with follow up resources and appointments needed.
  10. Ability to communicate with patients and staff daily in a courteous and professional manner, address questions and direct calls promptly.
  11. Demonstrate commitment to the patient and organization by good attendance, punctuality, professional behavior, and appearance.
  12. Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.
  13. Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.
  14. Additional duties or responsibilities as deemed necessary.
SKILLS AND ABILITIES:
Education:

Minimum education of LPN or medical experience equivalent in a medical setting.

Medical Assistant with one year or more related experience in medical/clinic setting

Experience:

One year of experience in medical field/setting, preferably case management.

Computer Skills:

Computer skills with capabilities to utilize Excel spreadsheets, graphs, and Word documents preferred. Ability to learn and utilize electronic record keeping systems such as electronic medical record required.

Other Requirements:

Willingness to attend and/or complete continuing education related to this position and /or any other topic deemed necessary to MHSI. Detail oriented, positive attitude, and the ability to take direction and work well with a team but also self-motivated.

The ability to work in a constant state of alertness and in a safe manner.

REPORTING RELATIONSHIP:

The Quality Improvement Coordinator reports to, is supervised, and evaluated by the Clinical Quality Manager, in consultation with the Clinical Quality Director and the Chief Quality/Compliance Officer.

EVALUATION:

The evaluation of work performance will be ongoing and will be carried out by the Clinical Quality Manager, in consultation with the Clinical Quality Director and the Chief Quality/Compliance Officer. It will include specific duties and responsibilities of this position description plus employee attitude and general working behavior. Formal evaluation will be performed annually but may be initiated at other times by the Clinical Quality Manager, the Clinical Quality Director, and/or Chief Quality/Compliance Officer.

This is a FLSA Non-Exempt position.
