Job description

For 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. The Office of Planning and Evaluation oversees the planning and evaluation needs of the agency’s 160 plus programs serving about 300,000 clients a year. Service areas include affordable housing; case management; clinics & rehabilitation; emergency assistance; food assistance; homelessness prevention; immigrant integration; children, youth and families; parish and community services; and senior services. STATEMENT OF THE JOB: The Quality Assurance Specialist is responsible for the oversight of incident reporting and review primarily but not exclusively in our behavioral health programs. As such, the Quality Assurance Specialist serves a vital role in the risk management functions of the agency. The Quality Assurance Specialist is responsible, in collaboration with the QA Manager, to create, implement, and support a comprehensive incident reporting and system in compliance with relevant regulations as well as internal requirements. * Develop, implement, and support an incident reporting and review program for programs without incident reporting regulations. This includes defining incidents, developing reporting and review requirements, and monitoring compliance with incident reporting and review program. * Ensure that incidents have been documented, investigated, and reviewed appropriately and within required time frames. * Notify supervisor and senior management of high risk incidents including, but not limited to, serious incidents which may be the result of staff actions, incidents which may involve media coverage, incident which pose a risk of harm to staff or other service recipients. * Document and disseminate minutes of incident review committee minutes. * Serve as primary liaison with NYS Justice Center * Create annual trend report for management and Board of Trustees * Conduct periodic training on incident review procedures * Adjust incident review procedures as needed in response to agency need and/or regulatory change * Assist QA Manager with client grievance reports and document programmatic response QUALIFICATIONS: * Masters Degree in Social Work required

*LMSW or LCSW required

* Prior experience managing incident reporting processes. * Prior experience conducting special investigations such as investigations of wrongful conduct and allegations of abuse. * Must possess skills and working knowledge of Microsoft Office, Excel and Access. * Ability to travel to multiple locations within the five boroughs as needed. * Must be able to occasionally lift and/or move up to 10 pounds.
