Job summary

American River Home Care is seeking a Quality Assurance LVN to join the Advanced Care at Home team. The role focuses on accuracy of physician orders and nursing documentation to support patient plans of care.

Ideal candidates are LVNs with clinical nursing experience and strong detail orientation. The position offers a Monday–Friday daytime schedule with potential for a hybrid arrangement as the program evolves.

Qualifications

• Current, unrestricted California LVN license.
• Graduate of an accredited vocational nursing/LVN program.
• Current CPR/BLS certification.
• 1–2 years of clinical nursing experience in home health, home care, acute, or post-acute care.
• Strong attention to detail and ability to review clinical documentation for accuracy and compliance.
• Strong written and verbal communication skills.
• Proficiency with electronic health records and Microsoft Office.

Responsibilities

• Review physician orders for accuracy, completeness, signatures, dates, and timely processing.
• Review nursing documentation to ensure alignment with the plan of care.
• Identify inconsistencies or missing documentation between care documents and orders.
• Review lines, tubes, drains, wounds, wound care, and IV documentation.
• Ensure wound care and IV documentation reflect ordered care.
• Communicate discrepancies to clinicians and leadership for follow-up.
• Identify trends and opportunities for staff education.
• Assist with chart audits, QA projects, and QAPI data collection.
