Job summary

HA Home Care is seeking a compassionate Licensed Vocational Nurse to provide direct patient care under the supervision of a Registered Nurse, following individualized plans of care, delivering treatments, and working closely with our interdisciplinary team to help patients recover safely and comfortably at home.

You will document all patient services accurately, educate patients and families on diagnoses and the plan of care, coordinate with the team, and support pain and symptom management

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current, active LVN license in California
• Minimum 1 year of nursing experience; home health or medical/surgical experience preferred
• Valid driver's license with insured, reliable transportation
• Strong verbal, written, and organizational skills

Responsibilities

• Provide direct patient care as defined under California's Nurse Practice Act
• Implement nursing care following comprehensive assessments and the plan of care
• Provide accurate, timely documentation of all patient services
• Educate patients and families on diagnosis, self-care, and the plan of care
• Coordinate with the interdisciplinary team on patient needs
• Support pain and symptom management, monitoring patient response to treatment
