Job summary

American River Home Care is seeking an experienced Licensed Vocational Nurse (LVN) to join our Advanced Care at Home (ACAH) team as a Quality Assurance LVN. The role focuses on reviewing physician orders, nursing documentation, and plans of care to ensure accuracy and compliance.

You will work closely with clinical leadership, perform chart reviews, identify documentation gaps, support quality improvement, and help prepare for accreditation.

Qualifications

• Current, unrestricted California LVN license.
• Graduate of an accredited LVN program.
• Current CPR/BLS certification.
• 1-2 years of clinical nursing experience in home health or related settings.
• Strong attention to detail and ability to review documentation for accuracy and compliance.
• Strong written and verbal communication skills.
• Ability to organize and prioritize daily workload and timelines.
• Comfortable with EHR and Microsoft Office.

Responsibilities

• Review physician orders for accuracy, signatures, dates and timely processing.
• Review nursing documentation to ensure alignment with active plans of care.
• Identify inconsistencies or missing documentation and communicate with clinicians.
• Review lines, tubes, drains, wounds, wound care, and IV documentation for accuracy.
• Confirm wound care and IV documentation reflect orders and frequencies.
• Assist with chart audits, QA projects, and QAPI data collection/reporting.
• Support accreditation readiness and CHAP survey preparation.
• Develop and revise quality/d documentation policies and procedures.

American River Home Care is seeking an experienced and detail-oriented Licensed Vocational Nurse (LVN) to join our Advanced Care at Home (ACAH) team as a Quality Assurance LVN.

About the Role

The ACAH Quality Assurance LVN plays an important role in ensuring that physician orders, nursing documentation, and patient plans of care are accurate, complete, and aligned.

Working closely with our Clinical Educator and clinical leadership team, you will review physician orders and nursing documentation for patients enrolled in our Advanced Care at Home program. You'll help identify documentation gaps, support timely corrections, recognize trends, and contribute to our ongoing quality improvement efforts.

This position is especially focused on documentation involving physician orders, plans of care, wounds, IV therapy, lines, tubes, and drains.

What You'll Do

Review physician orders for accuracy, completeness, signatures, effective dates, and timely processing.

Review nursing documentation to ensure it supports and aligns with the patient's active plan of care.

Compare documented care with physician orders and identify inconsistencies or missing documentation.

Review documentation related to lines, tubes, drains, wounds, wound care, and IV administration.

Confirm wound care documentation reflects the ordered frequency, products, and treatment approach.

Confirm IV documentation accurately reflects ordered medications, rates, and frequencies.

Communicate documentation or order discrepancies to clinicians, the Clinical Educator, and clinical leadership for follow-up.

Identify recurring documentation trends and opportunities for staff education.

Assist with chart audits, quality assurance projects, and QAPI data collection and reporting.

Support accreditation readiness and survey preparation, including CHAP requirements.

Assist with the development and revision of policies and procedures related to quality and documentation compliance.

Maintain patient confidentiality and HIPAA compliance at all times.

What We're Looking For

Required Qualifications:

Current, unrestricted California Licensed Vocational Nurse (LVN) license

Graduate of an accredited vocational nursing/LVN program

Current CPR/BLS certification

Minimum of 1-2 years of clinical nursing experience in home health, home care, acute care, or post-acute care

Strong attention to detail and ability to review clinical documentation for accuracy and compliance

Strong written and verbal communication skills

Ability to organize and prioritize a daily workload and meet required timelines

Comfortable working with electronic health records and Microsoft Office

Preferred Qualifications:

Previous home health experience

Experience with quality assurance, chart auditing, QAPI, or clinical documentation review

Experience reviewing physician orders and plans of care

Experience with wound care and/or IV/infusion documentation

Familiarity with CMS Conditions of Participation for Home Health

Experience with Axxess

QAPI or chart-auditing training or certification
