Salary Range: $46,800.00 (Min.) - $52,597.00 (Mid.) - $62,270.00 (Max.)

The Quality and Population Health Coordinator I (QPHC) is responsible for outreaching to members to close care gaps, outreach to providers for medical record pursuit and retrieval, and supporting L.A. Care medical groups by assisting with scheduling member appointments. This role combines remote and in-office work, primarily embedded in provider offices, providing direct outreach to members. The QPHC is vital to L.A. Care’s quality improvement efforts, aiming to improve health outcomes, population health, health equity, and member and provider experience.
Required: High School Diploma or equivalency.
Preferred: Associate's Degree.
Required: At least 3 months of experience with workflows related to patient or provider communication and outreach, and experience working in an office setting with basic equipment.
Preferred: Experience in health plan, provider practice, or health education settings.
Required: Compassion, strong motivation, excellent communication, interpersonal skills, proficiency in Microsoft Office, organizational skills, effective communication with patients and providers. Knowledge of HEDIS and quality measures, bilingual abilities are highly desirable.
Active & current driver’s license, auto insurance.
Light physical requirements, travel to offsite locations, and a limited duration position of 1-2 years with full benefits.
