Job summary

Gallipolis Developmental Center is seeking a Qualified Intellectual Disability Professional to lead habilitative programming for a caseload of 8-30 clients. You will chair interdisciplinary teams, supervise staff, and ensure documentation meets Medicaid ICF/IID standards.

The role requires clinical credentials and experience with developmental disabilities, with a strong emphasis on coordination, staff training and program evaluation.

Qualifications

  • At least one year of experience working directly with persons with intellectual disability or other developmental disabilities.
  • Doctor of Medicine or Osteopathy; or Registered Nurse; or Bachelor’s degree in professional human services.

Responsibilities

  • Coordinates overall development and implementation of client-based habilitative programming (IHP/IPP).
  • Acts as chairperson of inter-disciplinary team meetings to coordinate comprehensive service delivery and supervise staff.
  • Conducts periodic rounds to ensure proper implementation of programming and staff documentation and compliance.
  • Chairs team meetings to review 30/90 day and annual client habilitation plan reviews, goals, and minutes; supports mock surveys and policy updates.
  • Coordinates client home visits, transfers and discharges; participates in trainings; liaises with families, guardians and staff; prepares reports and correspondence.

JOB DUTIES IN ORDER OF IMPORTANCE:

  • Coordinates overall development & implementation of client-based habilitative programming (example, individual habilitation plan (IHP); individual program plan (IPP); individual behavior guidelines)

  • Acts as chairperson of inter-disciplinary team meetings in order to coordinate comprehensive delivery of services (example, speech, medical, pharmacy, social services, psychology, psychiatry, vocational, educational) and model appropriate behavior/intervention to assigned caseload of 8-30 clients, supervises assigned staff (example, residential care supervisors; hospital aide coordinators; therapeutic program workers; teachers; activity therapy specialists; juvenile correctional officers; social workers)

    in delivery of services, trains staff in effective delivery of training techniques/methods, ensures habilitative services & documentation meet requirements (example, Medicaid ICF/IID) & maintains accurate records of program implementation, reviews programs to determine effectiveness/compliance & initiates modifications/corrective action if necessary.

  • Conducts periodic rounds of cottages/modules/units/buildings or other active treatment locations to ensure proper
    implementation of programming & to monitor staff (example., therapeutic program workers) in proper documentation, appropriate client interactions & compliance with Medicaid ICF/IID regulations and standards

  • Acts as chairperson of team meetings to review, analyze and modify 30/90 day & annual reviews of client habilitation plans to include review of information prior to team meetings, chairing meetings, facilitation of discussion, development of goals and objectives for clients, prioritization of goals and objectives, proofreading minutes of meetings and writing of individual programs, changes to and/or modifications of client programs on ongoing basis and correction of deficiencies; participates in mock surveys of facilities; recommends proposed policies and procedures.

  • Coordinates client home visits, transfers (example, inter/intra facility) and discharges to ensure efficiency; acts as relief residential care supervisor and/or assists therapeutic program workers in training clients; attends meetings, training, workshops &seminars; serves on committees and task forces; acts as liaison with client families, guardians, case management staff and/or community-based staff; prepares reports, forms and correspondence related to clients and staff assigned; assists with coordination of youth admissions and releases.

Qualifications

Per Federal Register, Vol. 53. No. 107, Section 453.430, applicants must have at least one yr. of experience working directly with persons with intellectual disability or other developmental disabilities & is one of following.

  • Doctor of Medicine or Osteopathy
  • Registered Nurse
  • Individual who holds at least a bachelor’s degree in professional human services category (e.g., Psychology, Education, Anthropology).

  • Hourly wage will be paid at step 1, unless otherwise specified by collective bargaining agreement or rules outlined in the ORC/OAC.
