PROVIDENT PLACE in Apple Valley, CA seeks a dedicated Qualified Intellectual Disability Professional (QIDP) to oversee individualized care plans for residents with developmental disabilities across two licensed ICF facilities. You will lead ISP schedules and Human Rights Committee meetings, while supervising PMs, staff training, and ensuring regulatory compliance.

The role requires a Bachelor’s in Human Services or Social Sciences, CDPH rule knowledge, and strong management skills.

Qualifications

  • Bachelor’s degree in Human Services or Social Sciences.
  • Experience working with individuals with developmental disabilities in residential or home & community care settings.
  • Knowledge of CDPH rules and Regulations (Title 17 and Title 22); resident rights; CPR; background and TB tests.

Responsibilities

Job Summary

We are seeking a dedicated and compassionate Qualified Intellectual Disability Professional (QIDP) to join our dynamic team. In this vital role, you will oversee the development and implementation of individualized care plans for residents with developmental disabilities, ensuring they receive the highest quality of care in a supportive environment. You will provide leadership in the operations of two licensed Intermediate Care Facilities located in Apple Valley, CA.

Responsibilities:
  1. 1. Ensure all data collected and all Policies and Procedures followed.
  2. 2. Manage all ISP schedules and HRC meetings.
  3. 3. Monthly In-Services and implementation at each home.
  4. 4. Ensure Program Managers (PMs) In-Service the staff on
    • A. Cleanliness
    • B. Med Errors
    • C. Body Checks
    • D. Communication with Day Program Van Drivers and Day Program site staff.
    • E. Other
  5. 5. Ensure PMs are meeting the requirements of their job.
  6. 6. Ensure PMs are collecting all required data.
  7. 7. Coordinate consultant assessments and recommendations in-order-to formulate 30-day post, annual, and semi-annual ISPs as needed.
  8. 8. Schedule and conduct IDT meetings.
  9. 9. Creates, modifies, updates, and distributes Direct Care Staff schedules to PMs.
  10. 10. Work as Direct Care Staff as necessary including weekends, nights and, holidays when Direct Care Staff call-off work.
  11. 11. Ensure PMs create the work schedule with zero overtime hours each week.
  12. 12. Ensures PMs complete grocery shopping with clients each week.
  13. 13. Ensure client objective plans and data collections are implemented and recorded daily.
  14. 14. Ensure the ISP reports are distributed to day program, Regional Center case workers, Consultants, etc.
  15. 15. Coordinate services and participation with Regional Centers, Day Programs and other agencies.
  16. 16. Manage all hiring by attending job fairs (EDD, etc.), selecting applicants for interview, interviewing hiring candidates, completing/processing the new employee hiring package and orientation.
  17. 17. With Administrator approval, takes the lead in counseling and disciplinary actions up to and including termination.
  18. 18. Maintain client records at each home in accordance with regulatory requirements.
  19. 19. Complete monthly summaries of client data in a timely manner.
  20. 20. Complete All Special Incident Reports.
  21. 21. Notify verbally and submit written reports to the CA Dept. of Public Health and Regional Center and other interested parties for any reportable incidents.
  22. 22. Conduct and Chair Quarterly Human Rights meetings.
  23. 23. Conduct Quarterly Staff Meetings during which include discussion about mandated reporting/abuse.
  24. 24. Report to RN, Director of Operations and President for any Change of Condition, unusual occurrences and facility staffing issues.
  25. 25. Complete bi-annual HS231, Prolonged Care Assessment and MC171 forms for all clients, on time, to ensure client Medi-Cal billing is not cut-off.
  26. 26. Performs new client assessments by visiting potential new client placements and communicating with CSC regarding client history, background and medical diagnosis for possible immediate placement in ICF facilities.
  27. 27. Processes all new client admissions and discharges including new client assessment, admission paperwork and discharge form MC171.
Qualifications
  • Bachelor’s degree in the areas of Human Services / Social Sciences fields.
  • Proven experience working with individuals with developmental disabilities in residential or home & community care settings.
  • Knowledge of CDPH rules and Regulations (Title 17 and Title 22), knowledge of resident’s rights, CPR certification, pass criminal background test, pass physical and TB test.
  • Strong management skills with demonstrated supervisory experience in healthcare or residential care facilities.
  • Proficiency in Microsoft Office suite (Word and Excel) for documentation, reporting, and communication purposes.
