Job summary

New Horizons seeks a QIDP/Administrator to oversee operations of the ICF-DDH homes within the Residential Program. This role partners closely with the Director of Residential Services to coordinate resident care, services, and regulatory compliance.

The incumbent will supervise staff, ensure documentation accuracy, and maintain liaison with licensing agencies. Responsibilities include on-call coverage and development of ISP plans for residents.

Qualifications

  • One year experience working with the developmentally disabled.
  • Must pass a criminal background check.
  • Must have valid California driver's license and driving record acceptable.

Responsibilities

  • Oversee day-to-day operations of the ICF-DDH facilities and programs.
  • Oversee resident affairs including placement, behavior, health maintenance, and activities.
  • Coordinate with Counseling Dept, Regional Center, families, and staff for supervision and habilitation.
  • Maintain liaison with licensing agencies to ensure compliance with standards.
  • Write and submit plans of corrections; obtain approval from Director.
  • Oversee facility maintenance, food procurement and menu implementation.
  • Write residents' ISP and monthly progress notes.
  • Provide minimum 1.75 hours per week per resident of services.
  • Be on call 24/7 and coordinate with licensing officials as needed.
  • Attend conferences and submit program reports to the Director.

Position Type: Full-Time, Exempt

Schedule: Monday - Friday, 9 AM to 5 PM, followed by evening on-call duties from 5 PM onward and backup on-call coverage one weekend per month.

Salary: $70,720.00 per year

SUMMARY: In collaboration with the Director of Residential Services, oversees the management and the coordination of the operations of the ICF-DDH homes within the Residential Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Oversee and coordinate day-to-day operations of the ICF-DDH facilities and programs
  • Oversee resident affairs, which include but are not limited to, placement/termination, behavior intervention, skills training, financial resources, health maintenance, day programs, respite services, and recreational activities
  • Coordinate with the Counseling Department, Regional Center, resident family, ICF-DDH professionals, IPS/T, and direct care staff for effective resident supervision, active treatment, habilitation, health maintenance and evaluation
  • Maintain liaison with all monitoring agencies, such as Department of Public Health and Regional Center to ensure compliance with licensing and certification and other regulatory standards
  • Writes and submits all plans of corrections for all inspection findings. Ensure plans are reviewed and approved by Director of Residential Services before submission
  • Oversee home maintenance and repair, food purchase and menu implementation (with consultation from the facilities' dietician)
  • With the assistance of the IDT members, write the residents' Individual Service Plans
  • Write monthly resident progress notes
  • Recommend needed equipment and supplies for the program to the Director of Residential Services
  • Conduct monthly facility inspections to assure that all standards meet Department of Public Health licensing and certification and HUD specifications
  • Participate in mock surveys for all residential homes
  • Responsible for overseeing that the Resident Unit Records are properly maintained
  • Will provide a minimum of 1.75 hours per week, per resident, of needed services
  • On call 24 hours per day, 7 days a week
  • Designated facility representative for receiving licensing officials. Will contact Director of Residential Services upon arrival of licensing officials
  • Attend and/or conduct conferences with the educational and other service personnel related to the residents, as needed
  • Submit ICF program report/update to Director of Residential Services weekly
  • Visit and consult with residents' day program counselors monthly
  • Responsible for reporting immediately to SFVAR staff, licensing and placement agencies any unusual incidents that might be harmful to the health, safety and/or welfare of the residents, staff, and visitor, and submitting the corresponding incident report to the Residential Services Administrator
  • Monitor and coordinate social/recreational activities programs of residents. Coordinate corresponding vehicle use
  • Attend agency meetings as assigned by Director of Residential Services
  • Responsible for other duties as may be assigned from time to time
Responsibilities in Absence of Direct Support Staff
  • Assists with implementation of living skill development and personal care needs of the residents as established in the resident's individualized service plan (ISP). This may include the supervision and/or assistance of cooking, shopping, doing laundry, showering, feeding, dressing, toileting, and mobility (this may include lifting a resident from a seated or lying down position) as indicated by the resident's need
  • Assists in all active treatment programs as outlined in the ISP. Including assisting the resident with their behavioral development by implementing approved behavior plans and physical techniques when necessary to protect the resident and others
  • Administers medications to residents as directed by the facility's Registered Nurse and as prescribed by the resident's physician
  • Transports and accompanies residents to recreational outings (ie movies, musical, parks) and medical appointments using the agency van
SUPERVISORY RESPONSIBILITIES

Directly supervises designated employees in the Residential Department. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

In the absence of the Director of Residential Services, the QIDP/Administrator must be capable of pursuing a course that is consistent with the direction established by the SFVAR Board. The QIDP/Administrator must have an in-depth understanding of the organization's goals and the methods by which those goals must be realized. The QIDP/Administrator must be able to effectively fulfill the ethical and professional expectations of the position, as stated in the job description. They must be able to communicate effectively with management and professional staff outside of the facility, and must be a team builder and leader capable of providing cohesion and direction to other staff in the realization of the agency's purposes.

Must pass a criminal background check approved by the CA Dept of Health. Must have valid California driver's license and a driving record acceptable to the agency's insurance carrier. Must obtain agency driving certification within 90 days of employment and maintain certification in good standing while employed. Must have current CPR and First Aid certifications. Must also obtain agency medication certification within 90 days of employment and maintain certification in good standing while employed.

EDUCATION and/or EXPERIENCE

One year experience working with the developmentally disabled and one of the following: Master's degree in Psychology, Medical Doctor, Educator with a degree in Education, Physical Therapist, Occupational Therapist, Speech Pathologist, Audiologist, RN, Recreation Therapist, Rehabilitation Counselor, or a Bachelor's degree in the Human Services field.
