Job Summary

The QIDP is responsible for overseeing the care, development, and management of programs and services for individuals with intellectual and developmental disabilities (IDD). This position ensures that each individual habilitation plan is effectively implemented and meets all regulatory, health, and safety standards required by CMS ICF regulations. The QIDP leads the interdisciplinary team to assess, develop, and monitor Individualized habilitation plans that enhance the quality of life for individuals served.

Duties and Responsibilities
  • Through the assessment process, develops, implements, and oversees Individualized Habilitation Plan (IHP) for each individual served, ensuring person-centered goals and objectives are met.
  • Lead interdisciplinary team meetings to review and modify service plans based on individuals’ progress and changing needs.
  • Ensure compliance with all federal, state, and local regulations regarding the provision of services for individuals with intellectual and developmental disabilities.
  • Conduct assessments and evaluations to identify the strengths, needs, and preferences of individuals in the program.
  • Monitor and document the progress of individuals, maintaining detailed records and reports.
  • Provide training, guidance, and support to Direct Support Professionals (DSPs) to ensure the delivery of high-quality care.
  • Advocate for the rights and needs of individuals, promoting self-determination and community integration.
  • Coordinate with healthcare providers, family members, and other support personnel to address the physical, emotional, and social needs of individuals.
  • Encourage effective interactions between residential staff and clients in carrying out programs and managing behaviors.
  • Routinely observing clients across settings in program areas to assess effectiveness of program implementation and consistency of training effort to determine effectiveness of IHPs and PBSPs and making timely modifications to facilitate achieving desired skills or goals.
  • Routinely interacting with program staff across settings to assist in determining the effectiveness and continued relevance of program plans in meeting identified client needs.
  • Identifying inconsistencies in training approaches or programs not being implemented as written and facilitating the resolution of these inconsistencies.
  • Assures follow-up occurs for any recommendation for services, equipment or programs.
  • Present necessary IHP and PBSP changes to the IDT for approval.
  • Ensure daily, that all incident reports are completed accurately and timely, provide completed incident reports to Risk Management daily (M-F) after reviewing them daily in the Stand-Up Meetings. Submit follow up documentation on actions noted to Risk Management within designated timeframes
  • Make sure all necessary equipment per client IHP is purchased, in good repair, and available as indicated on the IHP and PBSP.
  • Attend all client related meetings for assigned case load.
  • Follow up on all recommendations presented by the Annual Review, Client Status, Behavior Management, PMR, any Ad Hoc committee or meeting, and Human Rights Committee.
  • As part of the transition process, facilitates transition-related education and activity. Provide information to the Administrative Coordinator for Medicaid reports and referrals.
  • Assist in training community provider staff during the transition process of individuals on assigned caseload.
  • Attend court hearings as needed for individuals involved in the court/legal system and inform the facility management of the results of each hearing.
  • Monitor throughout the month client observations to include but not limited to; mealtime, activities, job site, outings, med-pass, to ensure active treatment if provided on a 24-7 basis.
  • Develop 24-hour activity schedules and monitor for ICF compliance.
  • Monitor client monthly progress via notes and observations.
  • Provide QIDP Quarterly reports as required by CMS regulations or other more frequent interval as determined by the facility and Executive Director.

Qualifications

KNOWLEDGE, SKILLS AND ABILITIES

  • Bachelor’s degree in social work, psychology, special education, or a related field.
  • At least 1-2 years of experience working with individuals with intellectual and developmental disabilities.
  • Knowledge of federal and state regulations governing services for individuals with IDD.
  • Strong organizational, communication, and leadership skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Required to pass health screening, including a test for Tuberculosis, performed by or under the supervision of a physician not more than one year prior or seven days after employment.
  • Able to maintain confidentiality of patient information
  • Effective organizational and time management skills
  • Excellent written and verbal communication skills
  • Proficient in Microsoft Office
  • Ability to organize and manage multiple priorities
  • Ability to stand, sit, bend, lift, climb, balance, stoop, kneel, crouch, and move intermittently. Use hands to handle or feel. Ability to turn on the knees, back, wrist, shoulders, and grasp with both hands. Must be able to frequently lift/move up to 50lbs. and occasionally lift and move more than 50lbs.
  • Must obtain and maintain current certification in First Aid and Cardiopulmonary Resuscitation (CPR), including abdominal thrust technique. Training must be obtained through class attendance that includes hands-on instruction.
  • Must obtain and maintain current certification of Professional Crisis Management training (PCM).
