Qualifications

  • Strong interpersonal, verbal, and written communication skills.
  • Ability to obtain First Aid, CPR, Medication Administration, and other competencies within established timeframes.

Responsibilities

  • Ensure outcomes identified in Personal Plans and Implementation Strategies are addressed.
  • Complete annual assessments for individuals on the caseload.
  • Write annual Personal Emergency Preparedness Plans (PEPPs).
  • Obtain required intake and annual consents from individuals/guardians.
  • Maintain accurate charts and records for individuals on the caseload.
  • Train staff on Implementation Strategies upon hire and annually.
  • Monitor Direct Support Professional performance and address opportunities for improvement.
  • Complete QIDP audits twice monthly at assigned residential homes.
  • Respond to crises as required.

We are seeking a Qualified Intellectual Disabilities Professional (QIDP) – Implementation Specialist to support adults with developmental disabilities in a community-based residential setting in East Moline, Illinois. This role focuses on implementing person-centered services, completing assessments and required documentation, training and supporting direct-care staff, monitoring service quality, and coordinating with guardians and interdisciplinary team members.

Compensation

$62,000-$68,000 annually.

Position Overview

The QIDP Implementation Specialist coordinates and monitors supports for individuals living in community-based residential settings. The position works closely with individuals, guardians, Direct Support Professionals, Independent Service Coordinators, behavior analysts, nurses, supervisors, and other members of the care team to promote independence, safety, and quality services.

Key Responsibilities

  • Ensure desired outcomes identified in individuals’ Personal Plans and Implementation Strategies are being addressed.
  • Complete annual assessments for individuals on the assigned caseload.
  • Write annual Personal Emergency Preparedness Plans (PEPPs).
  • Obtain required intake and annual consents from individuals and/or guardians.
  • Maintain accurate charts and records for individuals on the assigned caseload.
  • Submit completed Implementation Strategies to guardians for signature.
  • Train staff on Implementation Strategies upon hire and annually.
  • Monitor Direct Support Professional performance and address opportunities for improvement.
  • Monitor documentation for completeness and accuracy and provide retraining when needed.
  • Assist with training new Direct Support Professionals.
  • Complete QIDP audits twice monthly at assigned residential homes.
  • Complete and upload discharge documentation when applicable.
  • Respond to crises as required or assigned.
  • Communicate with guardians, Independent Service Coordinators, behavior analysts, nurses, supervisors, and other members of the support team.
  • Attend appointments or accompany individuals to emergency room or urgent care visits as assigned.
  • Work with appropriate team members to help individuals obtain and maintain needed benefits.
  • Complete required documentation accurately and on time.
  • Participate in required meetings, continuing education, and training.

Qualifications

  • Bachelor’s degree in a Human Services field required.
  • At least one year of directly related professional experience required.
  • Valid driver’s license and current automobile insurance required.
  • Strong interpersonal, verbal, and written communication skills.
  • Ability to maintain accurate records and complete required documentation on time.
  • Ability to understand and implement individualized goals, interventions, and support plans.
  • Ability to obtain required First Aid, CPR, Medication Administration, and other required competencies or certifications within established timeframes.

Schedule & Work Environment

Work hours are assigned based on program needs. The QIDP must be able to respond to emergency situations as required. The position includes work in community-based residential environments and may involve supporting individuals experiencing challenging or aggressive behaviors.
