Qualifications

  • CPR, First Aid, CPI, and medication administration training must be completed within 4 months and renewed annually; state licenses/certifications as required.

Responsibilities

Schedule: Schedule is flexible to the jobs Demands (1 morning shift before breakfast, 1 late shift ending after dinner and 3 flex shifts) + on-call Monday-Friday & rotating weekend schedule

Position Summary

Reporting to the Director of Residential Services, the Residential Site Manager/QIDP is responsible for overseeing residential programs that provide individuals with disabilities the opportunity to live in a safe, supportive, and home-like environment while achieving the highest level of independence possible. This position manages all aspects of resident care, treatment planning, regulatory compliance, facility operations, financial oversight, and staff supervision.

The Residential Site Manager/QIDP assists residents with activities of daily living, including meal preparation, grocery shopping, housekeeping, laundry, personal hygiene, medication administration, transportation to appointments and community activities, and participation in recreational opportunities. The position ensures residents receive quality services that promote independence, dignity, and personal growth while maintaining compliance with all agency, state, and funding source requirements.

Responsibilities
  • Promote and uphold a person-centered approach to service delivery that supports resident choice, independence, and community inclusion.
  • Assist residents in developing daily living, communication, social, and self-advocacy skills through instruction, support, and encouragement. Ensure residents receive appropriate services and supports as identified in their individualized plans. Monitor resident progress and outcomes, making recommendations for program modifications as needed.
  • Complete and oversee required documentation, ensuring accuracy, timeliness, and compliance with regulatory standards.
  • Develop, implement, and maintain Individual Habilitation Plans (IHPs), Individual Service Plans (ISPs), Individual Active Treatment Plans (IATPs), and other required assessments and reports.
  • Oversee resident finances and personal funds in accordance with agency policies and interdisciplinary team recommendations.
  • Foster positive relationships with residents, families, guardians, case managers, and community partners to support quality outcomes and resident satisfaction.
  • Plan, coordinate, and supervise the delivery of services to ensure residents receive quality care that meets their individual needs. Participate in resident admissions, screenings, assessments, and placement decisions.
  • Serve as Chairperson of the Interdisciplinary Team (IDT) and facilitate meetings involving guardians, Independent Service Coordination (ISC) agencies, healthcare providers, and other stakeholders.
  • Ensure treatment plans are developed, implemented, reviewed, and revised in accordance with active treatment principles and regulatory requirements. Coordinate medical, dental, behavioral health, and other specialized services as needed.
  • Monitor resident health and wellbeing and respond appropriately to medical concerns, emergencies, and changes in condition.
  • Support and encourage family and guardian involvement in resident care and program planning.
  • Maintain a safe, clean, and welcoming residential environment that promotes comfort, dignity, and independence.
  • Enforce house rules, program regulations, agency policies, and resident rights.
  • Conduct routine safety inspections and ensure compliance with OSHA standards and company safety procedures.
  • Ensure appropriate use of Personal Protective Equipment (PPE) and infection control practices.
  • Investigate, document, and report accidents, injuries, incidents, illnesses, and unusual occurrences.
  • Participate in Human Rights and Behavior Management Committee activities as assigned.
  • Coordinate routine maintenance, repairs, and upkeep of the residence, grounds, furnishings, and equipment.
  • Report significant maintenance concerns and capital improvement needs to administration.
  • Manage site expenditures and ensure responsible use of agency resources. Review and approve purchases, maintain inventory controls, and monitor supply levels.
  • Submit receipts, invoices, purchase orders, and financial documentation in accordance with established procedures.
  • Assist with budget planning, financial reporting, and expense monitoring to ensure fiscal accountability.Identify and recommend capital improvement projects and operational enhancements.
  • Support administrative initiatives, audits, licensing reviews, accreditation surveys, and funding source inspections.
  • Assist with community relations and represent the organization professionally during meetings, site visits, and public events.
  • Ensure staff are properly trained on policies, procedures, resident support plans, medication administration, safety practices, and regulatory requirements.
  • Provide leadership and direction for assigned residential staff, fostering a culture of teamwork, accountability, and person-centered service.
  • Monitor staffing levels and schedules to ensure adequate coverage and quality service delivery.
  • Directly supervises approximately 8–12 employees.Recruit, interview, train, schedule, supervise, coach, and evaluate employees. Responsible for employee selection, training, scheduling, performance management, and overall team effectiveness.Address employee concerns, performance issues, and disciplinary matters in collaboration with Human Resources and agency leadership.Conduct performance evaluations and provide ongoing feedback, mentoring, and professional development opportunities. Promote employee engagement and maintain a positive work environment aligned with the organization's mission, values, and goals.
Requirements
  • Education: Bachelor’s Degree in Social Work, Psychology, Education, Rehabilitation, Recreation Therapy, Sociology, Nursing Degree or Bachelor’s Degree in a related field as approved by DHS. OR, if hired before 2009, 5 or more years DD experience and HS Diploma. Temporary assigned RSM candidate may also qualify with 5 or more years DD experience and HS Diploma. Employees without DHS approved degree and experience are required to have another QIDP qualified staff to review, approve and sign off on treatment plans, MPNs and staffingand cannot conduct DHS training.
  • Experience: Minimum of 1 year of experience working with people with developmental disabilities. 1+ years of supervisory experience
  • Certifications/Licenses: CPR, 1st Aid, Crisis Prevention Institute (CPI) and DSP training, including medication administration, is provided by the company must be successfully completed within the first 4 months to be certified. Passing CPR, 1st Aid, CPI and medication administration training is also required annually. Illinois Food Service and Sanitation certificate needed within the first 6 months and renewed as required.
  • Pre-Employment Tests: Must pass state required CASAS literacy assessment prior to employment.
  • Computer Skills: Proficient with Microsoft Word, Excel and Outlook.
  • Background Checks: Must pass criminal background check. Must pass various State and Federal registry checks. Must pass DCFS Abuse and Neglect Tracking System check Must pass driving history check and Company policy criteria, maintain valid driver’s license and be 21 or older to drive Company van (or 25 or older to drive 15-person-capacity van at some sites).
  • Driving: Use of personal insured vehicle for Company related travel is required.
