Job summary

STARC of Louisiana is seeking a dedicated professional to manage a caseload of clients and coordinate the implementation of each client’s service plan. You will supervise and monitor progress, recommend plan modifications, and may transport clients.

The role also supports direct staff during emergencies and staff absences. The position follows a 240-day calendar, with a Monday–Friday, 40-hour week and on-call weekend rotation.

Qualifications

  • Bachelor’s degree in human services or related field; at least 1 year of relevant experience with individuals with IDD.
  • Supervisory experience preferred.
  • Current CPR & First Aid certification.
  • Valid driver’s license.

Responsibilities

  • Manage a caseload and coordinate client service plans.
  • Lead interdisciplinary team meetings and plan annual reviews.
  • Monitor client progress and update plans as needed.
  • Provide supervision support to direct care staff as needed.
  • Maintain client records and complete reviews.
  • Attend client meetings and communicate with families.

JOB DUTIES SUMMARY:

Assigned a specific caseload of clients and is responsible for assuring integration of the various aspects of each client’s program. Coordinates, supervises, and monitors implementation of each client’s individual service plans. Responsible for recommending modification to individual service plans to accommodate client’s progress. May be required to transport clients. May be required to serve as backup to direct support staff in emergencies and as needed due to absence of scheduled direct support staff.

WORK SCHEDULE:

Follows 240-day Operational Calendar

Monday – Friday Minimum 40 hours

On Call weekly required with weekend rotation

ESSENTIAL FUNCTIONS:
  • Completes monthly and quarterly client records reviews/prepares summary of each client’s progress.
  • Participates in quarterly CPOC meetings to review client progress or goals listed in CPOC.
  • Works with support coordinators in planning meetings and securing additional services as needed
  • Reviews quality and quantity of contacts pertaining to follow-up on questions, concerns, comments relative to individual clients.
  • Reviews quality and quantity of community integration activities.
  • Recommends needed changes in client’s individualized plans.
  • Facilitates Interdisciplinary (I.D.) team meetings/PCP meetings
  • Conducts annual planning meetings.
  • Investigates injury and incident reports.
  • Conducts internal client records audits.
  • Participates in development of behavior management plans and ensures adherence to plans when approved by ID team.
  • Reviews progress notes weekly
  • Reviews EVV/GPS clock in and approves time via LASRS
  • Collaborates with family/guardian to determine client’s strengths and training needs and recommends strategies and training methodologies to ID team for incorporation into client’s individual service plans.
  • Assists with supervision of direct support staff and coordination or all aspects of waiver services provided to clients.
  • Attends client specific meetings as needed.
  • Meets with clients, families, educators, state, and regional officials and communicates with families on a regular basis.
  • Writes and updates annual service plans and signs as QIDP on required reports.
  • Understands and works with client’s budgets and provides financial training designed to permit individual to function as independently as possible.
  • Assists with ensuring that all direct support employees adhere to all mandated licensing requirements, guidelines, procedures and STARC policies.
  • May accompany clients to medical, dental, psychiatric, and other related appointments and informs Waiver Services Administrator or Director of Operations and other appropriate staff, by memo, of significant changes resultant of such appointments.
  • Coordinates with other programs/departments to ensure meeting client’s needs in accordance with client’s Plan of Care.
PHYSICAL REQUIREMENTS:

Administrative, Management, Technical, Clerical-Programs

Medium work involves exerting 20 - 50 pounds of force occasionally or 10 - 25 pounds of force frequently to lift, carry, push, pull, or otherwise move objects; or an amount greater than negligible and up to 10 pounds constantly to move objects. Involves sitting, walking, or standing frequently.

OTHER RESPONSIBILITIES:
  • Serves as backup for one or more other waiver services QIDPs in times of absences
  • Serves as public speaker and ambassador for STARC and the individuals STARC serves
  • Available to work flexible schedule (nights, weekends, holidays, emergency, and crisis situations)
  • Performs other duties as assigned
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
  • Bachelor’s degree in human services or related field and minimum of one (1) year of experience working directly with individuals with intellectual and developmental disABILITIES
  • Supervisory experience preferred
  • Current CPR & First Aid certification
  • Valid driver’s license
  • Ability to work at a fast-pace and to respond to stressful situations with control and assurance
  • Ability to be patient and tactful when dealing with individuals with intellectual and developmental disABILITIES
  • Ability to establish and maintain good working relationships with staff, families, clients, and other professionals
  • Ability to function independently, attention to details, work under pressure and prioritize multiple projects
  • Ability to meet deadlines
