Responsibilities

• Implement the Individual Service Plan/Implementation Strategy in cooperation with the consumer, the family, the residential program, and other professionals involved.
• Maintain consumer records to ensure events are recorded, files kept current, and confidential according to schedule with cooperating agencies.
• Provide individual support to consumers when needed.
• Analyze program data sheets and develop monthly progress notes.
• Serve as liaison with crew chiefs in the regular workshop to document individual functioning.
• Complete State of Illinois mandated training to receive and keep.
