  • Support CMS mortality/readmission monitoring and other core measures reporting.
  • Guide education and process changes to improve patient outcomes and reduce costs.

Job Summary

The Coordinator - Clinical Outcomes at North Mississippi Health Services is responsible for coordinating and overseeing outcomes management in accordance with hospital policies and procedures. They collaborate with various stakeholders to develop and implement evidence-based care methodologies, ensure compliance with regulatory standards, and analyze data related to patient care to improve clinical outcomes.

Coordinates Job Functions
  • Coordinates regulatory compliance activities including, but not limited to, quality process and outcome measures, value payment metrics, and disease specific certification.
  • Develops, implements, coordinates and evaluates Performance Improvement (PI) programs/projects related to quality processes, populations, and outcomes.
  • Provides leadership and expertise in assessment and action planning related to disease specific certification and/or evidence-based guidelines.
  • Develops strategic plans and goals in conjunction with nursing and physician leadership to promote population management and outcomes.
  • Concurrent and retrospective review process for quality metrics.
Reporting/Recordkeeping
  • Assists in the monitoring, evaluating, and reporting of CMS mortality and readmission rates, hospital acquired conditions, and patient safety indicators. Effectively meets/assists in monitoring, evaluating and reporting of core measures through Premier data base and concurrent chart reviews.
  • Oversees the collection and analysis of data related to the outcomes management.
  • Maintains oversight of registry data collection and reporting responsibilities associated with Get with the Guideline, public reporting, and other quality related data
  • Maintains provider specific data utilized in OPPE process and provider quality incentive programs
Outcomes Management
  • Utilizes scientific approach in the analysis of patient care issues, based on current, evidence-based guidelines.
  • Identifies and tracks significant process and outcomes measures and compares to internal and external benchmarks.
  • Identifies quality care measure set guidelines and updates, facilitates the readiness of care providers through process changes and education, promoting optimal patient outcomes in a cost-effective manner.
  • Provides oversight/analysis of population outcomes to strategically determine focus areas.
Performance Improvement
  • Assesses current PI processes and makes necessary recommendations.
  • Collaborates with patient care team to identify optimal processes and measures throughout continuum of care.
  • Develop structured care methodologies (order sets, algorithm, care paths).
  • Evaluates complex patient progress along continuum, intervening and facilitating optimal patient outcomes.
Liaison
  • Leads/facilitates/serves as Quality representative on various committees/teams and records committee work (examples include Performance Improvement, Information Technology, Infection Control, Patient Safety Council, numerous Sections, etc.).
  • Serves as NMHS liaison for statewide initiatives (examples include MHCA, MSPQC, etc.).
  • Promotes working relationships between all members of the health care team across NMHS.
Policy And Procedure
  • Collaborate with leadership, medical staff, nursing staff, ancillary staff, and the education department to develop and implement evidence-based guidelines and processes for multiple acute and chronic conditions.
Communication
  • Effectively and efficiently communicate assessments, recommendations, resolutions as needed.
Regulatory Knowledge
  • Adheres to NMHS/NMMS policies/procedures/guidelines.
  • Complies with applicable local/state/federal policies/procedures/guidelines.
  • Supports hospital accreditation and disease specific certification readiness and activities.
  • Prepares and coordinates disease specific certification.
  • Provides oversight for technical specifications for required measure reporting.
Education
  • Assists with development and implementation of educational programs for employees and medical staff based on trends in performance.
  • Collaborates with nursing and physician leadership to develop and implement internal educational programs.
  • Collaborates with CME and education department for quality related external education (Outcomes Conference, Spring Cardiology Conference, ASLS, etc.).
  • Trains and updates community hospital quality staff in data abstraction and entry for quality data.
  • Educates and trains as specified for disease specific certification and licensure.
