Qualifications

• Bachelor's degree in a social sciences field or related field.
• Understanding of mental illness and how it impacts functioning.
• Ability to engage therapeutically with individuals living with mental illness.
• Valid driver's license, auto insurance, and insurable driving record required.
• Willingness to travel throughout the assigned service area.
• Reliable transportation.

Responsibilities

• Coordinates assessments, treatment planning or crisis intervention services.
• Facilitates healthcare, behavioral health and other services and supports as part of care coordination.
• Provides training and facilitates linkages for the use of community resources.
• Monitors symptoms and service delivery to ensure client needs are met.
• Obtains or assist clients in obtaining necessary services for basic necessities and services.
• Assists clients in accessing self-help support groups and increasing social support networks.
• Provides appropriate interventions to aid clients in the restoration of daily living skills.
• Coordinates and assist in crisis management and stabilization.
• Collaborates and communicates effectively with treatment team for the coordination of care.
