Job summary

Community Health Alliance is seeking an experienced Case Manager (QBHS) to support children, adolescents, and adults living with mental health conditions by coordinating care, teaching daily living skills and fostering recovery.

This community-based role includes home visits, service coordination and collaboration with families and other supports, providing tailored interventions to promote recovery and independence.

Qualifications

  • Bachelor’s degree in social sciences or related field.
  • Understanding of mental illness and its impact on functioning.
  • Knowledge of how to therapeutically engage a person with mental illness.
  • Valid driver’s license, auto insurance, and insurable driving record.
  • Willingness to travel throughout the assigned service area.
  • Reliable transportation.
  • Understanding of the community mental health system.

Responsibilities

  • Coordinate assessments, treatment planning or crisis intervention services.
  • Facilitate healthcare, behavioral health and other services as part of care coordination.
  • Provide training and linkages to community resources.
  • Monitor symptoms and service delivery to meet client needs.
  • Assist clients in obtaining services for basic necessities.
  • Help clients access self-help support groups and increase social support.
  • Provide interventions to restore daily living skills.
  • Coordinate crisis management and stabilization.
  • Collaborate with treatment team for care coordination.
