Qualifications

Position Overview:

The QA Nurse LVN supports the facility’s Quality Assurance and Performance Improvement (QAPI) program and promotes compliance with applicable federal and California regulations, facility policies, and standards of nursing practice.

The QA Nurse works closely with the Director of Nursing, Assistant Director of Nursing, MDS, Infection Preventionist, Unit Managers, Staff Development Coordinator, and interdisciplinary team to identify clinical and operational trends, conduct audits, monitor compliance, and assist in developing and implementing corrective action plans.

This position supports both the facility's Post-Acute/SNF and Subacute areas, ensuring that residents receive safe, effective, and high-quality care.

• Quality assurance and performance improvement: Perform assigned quality assurance and performance improvement support activities under the direction of the Director of Nursing or designee, including routine audits, data collection, monitoring, and follow-up related to the facility’s QAPI program.
• Clinical record and documentation audits: Conduct audits of resident medical records to assess compliance with nursing documentation requirements.
• Regulatory and compliance monitoring: Assist nursing leadership with regulatory compliance monitoring activities by completing assigned audits, gathering requested information, and reporting observations related to CMS, California Department of Public Health (CDPH), facility policies, and other applicable requirements.
• Infection prevention and control: Collaborate with the Infection Preventionist and nursing leadership on infection prevention and control initiatives.
• Risk management and clinical monitoring: Assist with assigned clinical monitoring and risk-management support activities by reviewing records, tracking follow-up items, and promptly reporting potential concerns to nursing leadership.
• Post-Acute/SNF and Subacute quality monitoring: Conduct quality audits in both Post-Acute/SNF and Subacute units.
• Staff education and support: Assist nursing leadership and the Staff Development Coordinator by identifying observed education needs from assigned audits and quality trends and supporting approved education activities as directed.
• Data collection and reporting: Collect, organize, and submit assigned quality data and audit results to the DON, QAPI committee, or other designated leadership according to facility process.

Qualifications:

• Current active LVN license in California required.
• Previous experience in a Skilled Nursing Facility (SNF), Post-Acute, Subacute, or similar healthcare setting required.
• Previous experience with Quality Assurance, QAPI, clinical audits, regulatory compliance, or nursing documentation review preferred.
• Knowledge of CMS and California skilled-nursing facility regulations preferred.
• Strong understanding of nursing documentation and standards of care.
• Experience with incident/accident review, care plan monitoring, infection prevention, or risk management is highly desirable.
• Strong organizational and time-management skills.
• Excellent written and verbal communication skills.
• Strong attetion to detail and ability to identify discrepancies and compliance concerns.
• Ability to work collaboratively with nursing leadership and interdisciplinary teams.
• Ability to maintain confidentiality and handle sensitive resident and facility information appropriately.
• RAC-CT, Infection Prevention, QAPI, or other relevant certification or training is a plus.
