Qualifications

• 0-1 years of experience as an LPN in a healthcare setting.
• State licensure as LPN and CPR/BLS required.
• Ability to administer medications and participate in care plans.

Responsibilities

• Provide direct patient care under RN supervision.
• Monitor vital signs and assist with daily living activities.
• Administer medications and treatments as prescribed.
• Document patient care and interventions timely.
• Educate patients and families on care and discharge planning.
• Collaborate with interdisciplinary teams to ensure coordinated care.
• Support emergency response and code team activities.
