Job summary

Apploi is seeking a Pulmonary Care Coordinator to lead our facility's pulmonary program, delivering clinical excellence and driving resident outcomes. You will coordinate care with nursing, therapy, and respiratory staff to ensure individualized plans and timely interventions.

This role emphasizes education, communication, and the development of a robust pulmonology program within a long-term care setting.

Qualifications

• Current Registered Respiratory Therapist (RRT) license in good standing.
• Current BLS certification.
• Experience in skilled nursing, long-term care, rehabilitation, or post-acute care.
• Leadership, supervisory, or program development experience.

Responsibilities

• Complete pulmonary assessments and evaluations for residents enrolled in the pulmonary program.
• Monitor respiratory status and identify changes requiring intervention.
• Develop, implement, and update individualized pulmonary care plans.
• Coordinate respiratory interventions and serve as a primary pulmonary resource for the facility.
• Provide ongoing education, coaching, and support to nursing, therapy, and respiratory staff.
